Building healthcare around the patient, not the clinic

How can digital technologies support a hospital-at-home model to create a more efficient healthcare system?

Data shows that hospitals offering inpatient-level care at home achieve reduced costs, improved health outcomes, and enhanced patient experiences.

To help you achieve the promise of a digitally enabled hospital-at-home model, the Connected Health Collaborative Community (CHcc), co-hosted by the Digital Medicine Society (DiMe) and Consumer Technology Association (CTA), has launched its inaugural project, Advancing a Sustainable Hospital-at-Home Ecosystem at Scale, with UMass Chan Medical School’s Program in Digital Medicine as the project’s Impact Partner.

This project team is creating action-oriented, open-access resources that will: 

  • Establish reimbursement and ROI clarity around digitally-enabled hospital-at-home models to help you drive the highest impact for patients

  • Present high-quality care pathways to help address the challenges posed by clinician shortages, insufficient capacity and access, and unsustainable costs

  • Showcase examples of workflow and financial optimization, patient outcomes, and clinical decision-making
