How can digital technologies support a hospital-at-home model to create a more efficient healthcare system?

Data shows that hospitals offering inpatient-level care at home have experienced reduced costs, improved health outcomes, and enhanced patient experiences. Digital technologies are positioned to maximize the effectiveness and accessibility of this approach. 

The Connected Health Collaborative Community (CHcc), co-hosted by the Digital Medicine Society (DiMe) and Consumer Technology Association (CTA)—is launching its inaugural project, Advancing a Sustainable Hospital-at-Home Ecosystem at Scale, with UMass Chan Medical School’s Program in Digital Medicine as the project’s Impact Partner. A multi-stakeholder team will create action-oriented, open-access resources to help you achieve the promise of a digitally enabled hospital-at-home model.

This project will:

  • Establish reimbursement and ROI clarity around digitally-enabled hospital-at-home models that drive the highest impact
  • Align on defining and quantifying the return of value associated with successful programs to foster intentional growth and investment into hospital-at-home programs
  • Showcase examples of workflow and financial optimization, patient outcomes, and clinical decision-making

Working together to transform a sick care system into a healthcare system

The CHcc leverages interdisciplinary expertise and research to establish best practices and address fragmentation in connected health.

We have also formally requested FDA participation in this collaborative community. We eagerly anticipate the opportunity to collaborate with them and others in the ecosystem to work towards impactful advancements in connected health.

CHcc aligns and engages with other government programs including, but not limited to:

  • Veteran’s Health Administration’s Home-Based Primary Care and Whole Health programs
  • The Center for Medicare and Medicaid Services Acute Hospital Care at Home program
  • Hospital Without Walls initiative and Chronic Care Management resources
  • The U.S. Department of Housing and Urban Development’s Older Adult Home Modification Program (OAHMP)

Members of the CHcc are becoming part of a pivotal moment that shapes the future of healthcare, harnessing the power of digital innovation to create a connected, healthier world for all.
